Report vehcile-related motor vehicle accident (MVA) incidents, care, custody, control (CCC) events, or any identified vehcile damage.

NOTE: Please ensure that vehicle claims form is completed using the online form included or uploaded using the pdf/word version as part of submitting any claim.

 

Instructions

  1. Click on ‘Company Vehicle Accidents and Claims’ icon

  2. Click on ‘’Request Now’

  3. Enter job details including asset details, location description, any images etc

  4. Complete Online Claims Form or Upload Copy from File

  5. Click "Next" then review request details before clicking “Submit Now”
